It's obvious Frank wasn't allowed to join his wife or girlfriend visiting the gyno-doctor. There he would see a lot of other things than showing in this site. This is only a poor role-play. I know it is not allowed in the USA to make more realistic gyno-movies, but there are countries where they do! Like some East-European countries.
